Repair of triceps tendon avulsions or ruptures.

چکیده

After the repair of rupture or avulsion of the triceps tendon it is usual to immobilise the elbow in plaster for a few weeks. A method is described which obviates the need for plaster and permits early mobilisation of the elbow. Technique. The patient lies in the prone position with the injured arm resting on his chest and supported by an assistant. A straight posterior incision is made from about 10 cm above the tip of the olecranon to about 3 cm below it. The fascia is incised and the proximal end of the ruptured or avulsed triceps tendon identified. It usually includes a bony fragment and, if so, this is excised. A hole 3 mm in diameter is drilled transversely through the olecranon about 1 cm distal to its tip. One end ofa 4 mm wide Mersilen band (Ethicon Ltd, Scotland) is passed through the hole. This band, which will reinforce the suture, is brought proximally to lie beside the tendon at the level of the musculotendinous junction (Fig. 1). It is threaded through a large needle and a loop is made through the tendon about two-thirds ofthe distance from the tip of the olecranon to the musculotendinous junction. The needle is then passed transversely through the tendon and another loop is fashioned (Fig. 2). The elbow is held in the extended position and tension is applied to the reinforcing band in order to bring the proximal end of the tendon close to the tip of the olecranon or to the distal end when this is present. The two ends of the band are tied together as shown in Figure 2. The knot is secured with silk sutures. The proximal end of the divided tendon is then sutured to the distal end (or to the periosteum) with Dexon or silk sutures. The torn musculotendinous junction on both sides also is sutured with Dexon. Active movement of the elbow is begun on the first postoperative day, the Mersilen band being strong enough to allow immediate mobilisation. Material and results. The method described was used to treat nine patients whose ages ranged from 35 to 60 years. All of them had complete or nearly complete avulsions or ruptures of the tendon ; all but one had, in addition, an avulsed fragment of bone (Levy, Goldberg and Meir 1982). One patient was operated on 14 days after injury, the others within three days.

The distal triceps tendon footprint and a biomechanical analysis of 3 repair techniques.

BACKGROUND Anatomic repair of tendon ruptures is an important goal of surgical treatment. There are limited data on the triceps brachii insertion, footprint, and anatomic reconstruction of the distal triceps tendon. HYPOTHESIS An anatomic repair of distal triceps tendon ruptures more closely imitates the preinjury anatomy and may result in a more durable repair. STUDY DESIGN Descriptive and...

Distal triceps ruptures

Distal triceps ruptures are rare injuries due to the special anatomical features of the muscle and tendon-bone junction. This injury typically occurs at the tendon-bone junction due to an eccentric contraction of the muscle. The treatment is controversial, especially in partial ruptures; surgical repair is indicated for complete ruptures of the distal triceps tendon.Several repair techniques have...

Triceps Tendon Ruptures Requiring Surgical Repair in National Football League Players

BACKGROUND Complete triceps tendon ruptures are relatively rare in the general population but slightly more prevalent in professional football. One prior study found 11 complete ruptures over a 6-season period. HYPOTHESIS Triceps ruptures occur more commonly in football linemen due to forced elbow flexion during an eccentric contraction and may occur more commonly with the increasing size and...

Triceps brachii distal tendon reattachment with a double-row technique.

Case reports and small series have reported variable results regarding the treatment of choice for patients with triceps brachii tendon ruptures. Early surgical repair has been recommended for acute complete ruptures of the triceps brachii distal tendon to prevent late functional disability. However, controversy exists regarding the optimum surgical technique of reattachment. In addition, vario...

Case Report: Delayed Surgical Reconstruction with an Achilles Tendon Allograft for Triceps Tendon Rupture: Technique and Results

Triceps tendon injuries are relatively rare and represent less than 1% of all tendon injuries [1-3]. Ruptures typically occur following an eccentric force applied against an actively contracting triceps, but may occur following direct trauma [4-8].Tendon ruptures can occur due to intrinsic or extrinsic factors, and risk rupturing with a rapidly applied force [9-10]. The most common mechanism ty...
